$2 million gene therapy cures require a financing model

A new generation of gene therapies can cure devastating diseases in a single dose, but their price tags of two million dollars or more are creating a crisis in healthcare financing. William Padula, a health policy expert, argues that the real problem is not a shortage of medical breakthroughs, but a lack of systems to pay for them.

"We do not lack cures. We lack the infrastructure to pay for and deliver them," Padula writes. He points out that while these treatments can eliminate lifelong chronic illness, their upfront cost is far beyond what most patients or insurers can handle. Traditional insurance models, which spread costs over years of premiums, are not built for a single, massive payment.

Padula proposes a shift toward "outcomes-based" financing. In this model, drugmakers would be paid in installments over time, with payments tied to the patient's continued health. If the cure fails, the payments stop. This spreads the financial risk and makes the high price more manageable for insurers and government programs like Medicaid.

Another idea is the creation of special reinsurance pools or government-backed loans that spread the cost across the entire population. Without such changes, Padula warns, the healthcare system will face a choice: deny access to life-saving cures or face financial collapse. The science is ready, but the checkbook is not.
